First Line Antibiotics For A Uti

Antibiotic Awareness: Urinary Tract Infection (UTI), Cystitis or Bladder Infection
  • Ampicillin
  • Nitrofurantoin
  • Trimethoprim/sulfamethoxazole

Notably absent from the list of antibiotics prescribed for the treatment of UTIs is Amoxicillin. While very popular and useful in treating numerous other bacterial infections, urinary tract infections are not amongst the infections Amoxicillin is used for.

Does Cranberry Juice Prevent A Uti

Some patients may want to use cranberry or cranberry juice as a home remedy to treat a UTI. Cranberry juice has not been shown to cure an ongoing bacterial infection in the bladder or kidney.

Cranberry has been studied as a preventive maintenance agent for UTIs. Studies are mixed on whether cranberry can really prevent a UTI. Cranberry may work by preventing bacteria from sticking to the inside of the bladder however, it would take a large amount of cranberry juice to prevent bacterial adhesion. More recent research suggests cranberries may have no effect on preventing a UTI

  • According to one expert, the active ingredient in cranberries — A-type proanthocyanidins — are effective against UTI-causing bacteria, but is only in highly concentrated cranberry capsules, not in cranberry juice.
  • However, cranberry was not proven to prevent recurrent UTIs in several well-controlled studies, as seen in a 2012 meta-analysis of 24 trials published by the Cochrane group.
  • While studies are not conclusive, there is no harm in drinking cranberry juice. However, if you develop symptoms, see your doctor. Some people find large quantities of cranberry juice upsetting to the stomach.

Increasing fluid intake like water, avoiding use of spermicides, and urinating after intercourse may be helpful in preventing UTIs, although limited data is available.

Is It Possible To Prevent Recurrent Urinary Tract Infections With A Vaccine

Currently, there are no commercially available vaccines for UTIs, either recurrent or first-time infections. One of the problems in developing a vaccine is that so many different organisms can cause infection a single vaccine would be difficult to synthesize to cover them all. Even with E. coli causing about most infections, the subtle changes in antigenic structures that vary from strain to strain further complicates vaccine development even for E. coli. Researchers are still investigating ways to overcome the problems in UTI vaccine development.

Can You Treat A Uti Without Antibiotics

Antibiotics are an effective treatment for UTIs. However, the body can often resolve minor, uncomplicated UTIs on its own without the help of antibiotics.

Complicated UTIs will require medical treatment. These UTIs involve one or more of the following factors:

More severe risks of using antibiotics include:

How Does Cipro Work

NGU (Non

Cipro is an antibiotic in the class of fluoroquinolones. This type of antibiotic is bactericidal. This means that it directly kills bacteria. It does this by blocking enzymes that are needed for bacteria survival.

Cipro is a broad-spectrum antibiotic. This means it works against many different types of bacteria. However, many bacteria have grown to be resistant to Cipro. Resistant bacteria can no longer be treated with a certain drug.

  • Alcohol should be avoided because metronidazole and alcohol together can cause severe nausea, vomiting, cramps, flushing, and headache.
  • Metronidazole can increase the blood thinning effects of warfarin and increase the risk of bleeding probably by reducing the breakdown of warfarin.
  • Cimetidine increases blood levels of metronidazole while cholestyramine reduces blood levels of metronidazole by reducing its absorption.
  • Metronidazole should not be combined with amprenavir for treating human immunodeficiency disease because amprenavir contains propylene glycol.
  • Metronidazole blocks the breakdown of propylene glycol in the liver leading to accumulation of propylene glycol in blood. Accumulation of propylene glycol could cause seizures, increased heart rate, and lead to kidney failure.
  • Metronidazole increases the blood levels of carbamazepine , lithium and cyclosporine though unknown mechanisms. Serious reactions may occur if these drugs are taken with metronidazole.

Antibiotics Used For Uncomplicated Utis

If you are a healthy individual whose urinary tract is anatomically and functionally normal and you have no known heightened UTI susceptibility youve got whats dubbed an uncomplicated UTI, according to guidelines published in August 2019 in the Journal of Urology. For these individuals, antibiotics are considered the first-line of treatment.

The type of antibiotics you are prescribed and for how long is contingent on the type of bacteria detected in your urine, your current health status, and whether your UTI is uncomplicated or complicated. Depending on which antibiotic your doctor prescribes, women may need a single dose or up to a five-day course. For men, antibiotics are usually given for a slightly longer period of time, notes UpToDate.

Typically, if you are diagnosed with an uncomplicated UTI, one of the following will be prescribed as first-line treatment:

The following antibiotics are considered second-line treatments for UTI. They are generally chosen because of resistance patterns or allergy considerations:

Urinate When The Need Arises

Frequent urination puts pressure on bacteria in the urinary tract, which can help to clear them out.

It also reduces the amount of time that bacteria in the urine are exposed to cells in the urinary tract, reducing the risk of them attaching and forming an infection.

Always urinate as soon as possible when the urge strikes to and treat UTIs.

Preventing Uti In Puppies

Why is it important to treat UTIs with antibiotics?

As with most medical conditions, prevention is always better than treatment. Though there is no hard and fast prevention for urinary tract infections in puppies, you can try the following to help avoid them:

  • Drink Water: Encourage your puppy to drink plenty of water by ensuring that a full bowl of clean and fresh water is always available.
  • Cool It Down: On hot days, toss some ice cubes in your puppys bowl to help cool the water down.
  • Use Flowing Water: Many dogs and puppies like to drink from running water. If your puppy is particularly adverse to a bowl, consider purchasing a pet drinking fountain.
  • Give Probiotics: If your puppy is experiencing chronic UTIs, ask your vet if probiotics might help.
  • Keep It Clean:Be sure to frequently clean your puppys bowls to keep them clear of bacteria and mold. This is especially important when your leave food and water out all day for your puppy to consume freely.
  • Switch Food: Again, if your puppy is prone to UTIs, talk to your vet about switching to a dog food made specifically to promote urinary tract health.
  • Get a Checkup: An important step in keeping your puppy healthy and happy is to have annual checkups, which will help avoid health issues like UTIs.

What Oral Antibiotics Are Used To Treat An Uncomplicated Uti In Women

The following oral antibiotics are commonly used to treat most uncomplicated UTI infections :

Your doctor will choose your antibiotic based on your history, type of UTI, local resistance patterns, and cost considerations. First-line options are usually selected from nitrofurantoin, fosfomycin and sulfamethoxazole-trimethoprim. Amoxicillin/clavulanate and certain cephalosporins, for example cefpodoxime, cefdinir, or cefaclor may be appropriate options when first-line options cannot be used.

Length of treatment for cystitis can range from a single, one-time dose, to a course of medication over 5 to 7 days. Kidney infections may require injectable treatment, hospitalization, as well as a longer course of antibiotic, depending upon severity of the infection.

Sometimes a UTI can be self-limiting in women, meaning that the body can fight the infection without antibiotics however, most uncomplicated UTI cases can be treated quickly with a short course of oral antibiotics. Never use an antibiotic that has been prescribed for someone else.

In men with symptoms that do not suggest a complicated UTI, treatment can be the same as women. In men with complicated UTIs and/or symptoms of prostatitis are not present, men can be treated for 7 days with a fluoroquinolone . Tailor therapy once urine cultures are available.

How To Take Cipro

Take Cipro exactly according to your doctors instructions. You might start to feel better before you finish your entire Cipro treatment. Even if you start to feel better, dont stop taking Cipro. In many cases, its important to finish the entire treatment to make sure the infection doesnt come back.

If youre feeling better and want to stop Cipro early, be sure to talk with your doctor first to make sure its safe to do so.

What Are Urinary Tract Infection Risk Factors

There are many risk factors for UTIs. In general, any interruption or impedance of the usual flow of urine is a risk factor for a UTI. For example, kidney stones, urethral strictures, an enlarged prostate, or any anatomical abnormalities in the urinary tract increases infection risk. This is due in part to the flushing or washout effect of flowing urine in effect, the pathogens have to “go against flow” because the majority of pathogens enter through the urethra and have to go retrograde to reach the bladder, ureters, and eventually the kidneys. Many investigators suggest that women are far more susceptible than men to UTIs because their urethra is short and its exit is close to the anus and vagina, which can be sources of pathogens.

People who require catheters have an increased risk as the catheter has none of the protective immune systems to eliminate bacteria and offers a direct connection to the bladder. Catheters that are designed to reduce the incidence of catheter-related infections are available , but are not used by many clinicians because of short-termed effectiveness, cost, and concern about antibiotic resistancedevelopment in bacteria.

How Vets Diagnose Utis In Cats

CAN flagyl treat a urinary tract infection?

Vets diagnose urinary tract infections by collecting a urine sample and examining it for the presence of bacteria. These samples must be collected through a process called cystocentesis, where urine is drawn directly from the bladder with a fine needle. This is necessary to prevent contamination of the urine sample from cups, tables, or any other surface the urine may come into contact with.

After cystocentesis, your veterinarian will run a culture-and-sensitivity test. During this test, the bacteria are isolated and studied. This helps the vet determine the best antibiotics for your cats exact infection.

Occasionally, acute infectionsespecially first-time infections that show up suddenlyare treated with a broad-spectrum antibiotic without trying to determine which bacteria are involved.

Chronic infections, however, almost always require more testing to isolate the type of bacteria causing the problem and identify the best antibiotic.

Although evidence of its effectiveness is limited, cranberry juice is widely used to prevent urinary tract infections . However, recent studies have shown that cranberry juice may affect the way the body absorbs and metabolizes antibiotics used to treat UTIs. This study will determine whether concurrent cranberry juice administration lowers the urinary concentrations of two commonly used antibiotics, amoxicillin and cefaclor.

This study will involve children being treated for UTIs and adults without UTIs. All of the children will continue their prescribed amoxicillin/cefaclor treatment. After 7 to 10 days of antibiotic treatment, the children will be assigned to receive either cranberry juice for 2 days or no additional treatment. Urine samples will be collected from all child participants before and after the administration of cranberry juice to examine the excretion of the antibiotics.

How Long Does It Take To Work

Cipro begins to work against bacterial infections within hours of when you take it. However, you may not notice improvement in your symptoms for a few days.

The Cipro dosage your doctor prescribes will depend on several factors. These include:

  • the type and severity of the condition youre using Cipro to treat
  • your age
  • the form of Cipro you take
  • other medical conditions you may have, such as kidney disease

Typically, your doctor will start you on a low dosage and adjust it over time to reach the dosage thats right for you. Theyll ultimately prescribe the smallest dosage that provides the desired effect.

The following information describes dosages that are commonly used or recommended. However, be sure to take the dosage your doctor prescribes for you. Your doctor will determine the best dosage to suit your needs.

Other Antibiotics And Treatments For Urinary Tract Infections

Other antibiotics may be as effective as first-line antibiotics but have more side effects or risks of complications. They are not commonly used. They include:

  • Fluoroquinolones
  • Ciprofloxacin
  • Levofloxacin

Antibiotics called beta-lactams may be used when other first-line antibiotics are unavailable or cannot be used for any other reason. They include:

  • Amoxicillin-clavulanate
  • Cefdinir
  • Cefaclor

These are not usually first-line choices because they are broad-spectrum antibiotics that have a higher risk of causing antibiotic resistance.

Another drug that is frequently prescribed for a UTI is phenazopyridine, available under several brand names such as Pyridium. This medication is not an antibiotic and does not cure a UTI. It is used to relieve symptoms of pain, burning, urgency and pressure.

Antibiotic Warnings And Treatment Concerns

Ask Dr. Nandi: Antibiotic-resistant urinary tract infections are on the rise

The most commonly prescribed antibiotics for uncomplicated UTIs are similar in efficacy. But its important to note that ampicillin, amoxicillin, and sulfonamides are no longer the drugs of choice for combatting UTIs because of the emergence of antibiotic resistance. In addition, amoxicillin and clavulanate has been shown in previous research to be significantly less effective than others when it comes to treating urinary tract infections.

Also, as noted above, the FDA advises against using fluoroquinolones for uncomplicated UTIs. These medicines should only be considered if no other treatment options are available. In some cases, such as a complicated UTI or kidney infection, a healthcare provider may decide that a fluoroquinolone medicine is the best option, notes the American Academy of Family Physicians.

For pregnant women, some common antibiotics, such as fluoroquinolones and tetracyclines, should not be prescribed because of possible toxic effects on the fetus. But oral nitrofurantoin and cephalexin are considered good antibiotic choices for pregnant women with asymptomatic bacteriuria and acute cystitis, according to past research.

Are There Any Over

Over-the-counter antibiotics for a UTI are not available. You should see your doctor to have your symptoms evaluated.

Your provider may recommend an OTC product called Uristat to numb your bladder and urethra to ease the burning pain during urination. Uristat can be bought without a prescription at the pharmacy. A similar phenazopyridine product called Pyridium is also available.

Take phenazopyridine for only 48 hours, and be aware it may cause your urine to turn a brown, orange or red color which may stain fabrics or contact lenses. It may be best to not wear contact lenses while being treated with phenazopyridine.

Phenazopyridine is not an antibiotic and will not cure a UTI.

Treatment For Utis In Cats

If you see any signs of UTI in your cat, take them to the vet. For male cats, it can be fatal to wait even a few hours. For female cats, feed them canned food only and see the vet within 24 hours. You should not try to treat the UTI at home with things like cranberry products, which have not been proven to be effective for UTIs in cats.

Urinary tract infections in cats are usually treated with antibiotics. Often, vets also recommend feeding an all-canned diet for a period of time since dry foods often result in a more-concentrated urine, which can lead to urinary tract problems.

If your cat is particularly uncomfortable, your vet may prescribe anti-inflammatory pain medicationespecially if treatment is delayed a few days while you wait for test results. Acute infections usually receive a relatively short course of antibiotics, while chronic UTIs may require longer antibiotic use.

Cipro And Other Medications

UTIs and BV During Pregnancy: Risks to the Baby

Below is a list of medications that can interact with Cipro. This list does not contain all drugs that may interact with Cipro.

Before taking Cipro, be sure to tell your doctor and pharmacist about all prescription, over-the-counter, and other drugs you take. Also tell them about any vitamins, herbs, and supplements you use. Sharing this information can help you avoid potential interactions.

If you have questions about drug interactions that may affect you, ask your doctor or pharmacist.

Antacids

Many antacids contain calcium carbonate, magnesium hydroxide, and aluminum hydroxide. These ingredients can bind to Cipro and prevent your body from absorbing it. This can decrease how well Cipro works.

To avoid this interaction, take Cipro at least two hours before taking an antacid, or six hours afterward.

Anticoagulant drugs

Taking Cipro with oral anticoagulant drugs such as warfarin might increase the anticoagulant effects. This might result in increased bleeding. If you take an anticoagulant, your doctor may need to monitor your bleeding risk more frequently if you take Cipro.

Drugs that prolong the QT interval

Certain medications prolong your QT interval, which means they might affect the rhythm of your heartbeat. Taking Cipro with these drugs can increase the risk of having a dangerous irregular heartbeat. Cipro should be avoided or used very carefully with these medications.

Examples of these medications include:

Clozapine

Diabetes drugs

Methotrexate

Probenecid

Ropinirole
