Cranberries are the fruit that have been studied most for their usefulness in treating urinary tract infections. As noted in a 2013 issue of âReviews in Urologyâ, the clinical evidence for cranberry’s usefulness in promoting a healthy urinary tract is conflicting. The positive effects observed are believed to be due to cranberry’s high proanthocyanidin content.
You’ll find this powerful antioxidant at high concentrations in some common foods â but unfortunately, cherries aren’t anywhere near the top of the list. To illustrate that point, the best analysis of proanthocyanidin content in common foods was published in the March 2004 âJournal of Nutritionâ.
In it, the authors reveal that cherries have just 8.2 milligrams of proanthocyanidin content per 100 grams of fruit. Compare that to the findings for cranberries: Those tart little fruits have a whopping 418.8 milligrams of proanthocyanidin content per 100 grams of fruit â more than 50 times the content you’ll find in cherries.
The âReviews in Urologyâ analysis also notes that vitamin C is often recommended as a supplement to prevent recurring urinary tract infections â although the authors also note that there’s a lack of strong clinical evidence to support this claim in healthy, adult women.
Cranberry Juice And Combinations Of Its Organic Acids Are Effective Against Experimental Urinary Tract Infection
The antibacterial effect of cranberry juice and the organic acids therein on infection by uropathogenic Escherichia coli was studied in an experimental mouse model of urinary tract infection . Reduced bacterial counts were found in the bladder of mice drinking fresh cranberry juice. Commercially available cranberry juice cocktail also significantly reduced bacterial populations in the bladder, as did the hydrophilic fraction of cranberry juice . Quinic, malic, shikimic, and citric acid, the preponderant organic acids in cranberry juice, were tested in combination and individually. The four organic acids also decreased bacterial levels in the bladder when administered together , and so did the combination of malic plus citric acid and malic plus quinic acid . The other tested combinations of the organic acids, and the acids administered singly, did not have any effect in the UTI model. Apparently, the antibacterial effect of the organic acids from cranberry juice on UTI can be obtained by administering a combination of malic acid and either citric or quinic acid. This study show for the first time that cranberry juice reduce E. coli colonization of the bladder in an experimental mouse model of urinary tract infection and that the organic acids are active agents.

Cranberries contain a higher amount of total phenols per serving than other common fruits including blueberries , apples , and red grapes . Total phenols in cranberry juice ranged from 0.13 to 1.7 mg/mL . Total polyphenol content of cranberry products was higher for cranberry powder , dried cranberries , and frozen cranberries than for cranberry sauce and jellied sauce . A study on dried fruits ranked dried cranberry in the top three in terms of total phenol content of the six dried fruits examined, including raisins, figs, and plums with a content of 870 mg catechin equivalent per 100 g fresh weight .
Most of the phenolic acids present in the fruit are also present in cranberry juice, but at smaller quantities . Cranberry juice has an increased concentration of benzoic acid compared to other phenolic acids, ranging from 0.14 g/mL of caffeic acid to 5.11 g/mL of sinapic acid.

Probably Not A Great Treatment For Active Utis
While cranberry products help protect against UTI recurrence in some people, evidence supporting the use of cranberry juice and cranberry juice products for improving symptoms in people who have active UTIs is weak.
One review that included three high quality studies concluded that, overall, there wasnt enough evidence to show that cranberry extract helps treat active UTIs .
Another study that included 46 women found that taking cranberry capsules both alone and when combined with antibiotics may help reduce the need for antibiotic use and improve certain UTI-related symptoms in women with active UTIs .
Its important to note that this was a feasibility study with 46 participants, designed to assess whether a larger scale study would be feasible. Thus, its results may not be as robust as the results of a larger, high quality study.
Some women in the study noted that taking the cranberry supplements helped reduce antibiotic use and helped clear the infection more quickly than antibiotics alone, while others reported no improvement when they took the cranberry supplements.
Its important to note that most available research focuses on using cranberry products to prevent UTIs, not treat active infections.
Currently, theres not enough evidence to suggest that cranberry products are effective at reducing UTI symptoms or speeding recovery from active UTIs.
