But Which Laser Treatment Is Best For Urinary Incontinence Treatment

  • Laser Wavelength: Erbium: YAG laser is more skin-friendly in terms of ablative agents for the skin tissues since Er:YAG laser only generates 2,940 nm, while CO 2 laser in non-ablative mode offers a much longer wavelength of 10,600 nm.
  • Collagen Production: A CO 2 laser is more effective in terms of boosting collagen and elastin production as compared to an erbium YAG laser. Histologically, clinical trials show that stimulation of collagen and elastin using CO 2 laser ranges from 50 to 200 µm while only 30 to 50 µm is stimulated by Er:YAG laser. Thus, the effects of collagen stimulation last longer using a fractional CO 2 laser.
  • Thermal Damage: Er:YAG laser offers patient-specific thermal damage control, hence, offering a virtually pain-free procedure that does not require the application of any anaesthetics. Comparatively, a fractional CO 2 laser may require anaesthesia to ensure a comfortable and painless procedure. CO2 lasers are controlled by two modes, ablative and non-ablative. Thus, may cause longer and moderate downtimes as compared to the use of an Er:YAG laser.
  • Recovery Time: Recovery time for Er:YAG laser therapy is one week, while it may take between one to three weeks for a fractional CO 2 laser therapy.

Why Does Laser Therapy Help Female Incontinence

The two most common types of urinary incontinence are stress incontinence and urge incontinence.

Stress incontinence is the involuntary loss of urine with coughing, laughing and lifting. When you bounce on a trampoline with your granddaughter or daughter, this is stress incontinence. It is most often caused by urethral hypermobility a fancy doctor term for the urethra is no longer well supported due to child birth and/or aging.

The main support system to the urethra is collagen. Collagen is the primary protein supporting our tissues. Collagen levels have been found to be lower in women with stress urinary incontinence. Low female hormone levels post-menopause play a role as well.

Laser therapy for female incontinence increases collagen levels in the pelvic tissues supporting the urethra. In other words, thereâs actual science to support the use of laser therapy for female incontinence.

The laser energy and heat stimulates the lining of the vagina to contract the existing collagen . Laser therapy for female incontinence also stimulates the production of new collagen, blood vessels, and helps to thicken the lining of the vagina.

These internal changes lead to side benefits of laser therapy for female incontinence. In fact, most women perform laser therapy for these âvaginal rejuvenationâ benefits, not for urinary incontinence:

  • Improved vaginal moisture
  • Less pain during intercourse
  • Vaginal tightening
  • More enjoyable sex.

What Is Stress Incontinence

Incontinence is the leakage of urine from the bladder. This can occur for several reasons but the most common cause is a weakness of the stretched vaginal wall underneath that supports it.

There are several types of incontinence but the commonest cause is Stress Incontinence, where bladder leakage occurs with many things that cause an increase in pressure across the pelvis. Commonly such things as a cough, a sneeze, a laugh, exercise or straining.

Does Laser Therapy For Urinary Incontinence Work

This is the million dollar question. To date, the only published data supporting the use of laser therapy for urinary incontinence is with the Er:YAG laser. All other reports are anecdotal or made by providers who make money off the devices.

An unbiased review article cites multiple studies showing the efficacy of the Er:YAG laser in treating urinary incontinence.

Since most laser therapies are primarily performed for âvaginal rejuvenationâ, improvement in urinary incontinence is often a secondary benefit that has not been formally studied.

Radiofrequency ablation specifically for urinary incontinence has been evaluated in a pilot study. However, this device was delivered only through the urethral opening and would not be used for âvaginal rejuvenation.â

Female Urinary Incontinence Is An Untreated Condition For 50% Of Sufferers

As many as one in four of all sexually active women suffer some form of urinary incontinence, a debilitating, restrictive condition, particularly in women who have experienced vaginal childbirth. Contrary to common perception, urinary incontinence is not just an issue for older women who have lost muscle tone through ageing, but for a large and often silent majority of younger women who often silently accept the condition as a consequence of childbirth. Stretching and loss of elasticity, together with damage to the connective tissue of the pelvic floor, causes degrees of incontinence.

What Is The Best Laser Treatment For Urinary Incontinence

While surgery can be an option as a treatment of urinary incontinence, it can also be helpful to explore non-surgical or minimally invasive treatments such as laser-based procedures or radiofrequency techniques. In this fast-paced world that people thrive in, choosing a time and cost-efficient treatment that may yield the same or even better results as surgical procedures can be more gratifying.

Here are 2 laser techniques that are proven worthy of trying by patients with urinary incontinence:

Er:YAG Laser Therapy: Erbium: YttriumAluminumGarnet or Er: YAG is a solid-state laser comprising of an Er-YAG crystal that releases a wavelength of 2,940 nanometers. This erbium-doped yttrium-aluminium-garnet laser is a non-ablative technique that involves a photothermal stimulation of collagen neogenesis.

Fractional CO2 Laser: This type of laser therapy involves the use non-ablative active laser medium which consists of a mixture of 3 gases consisting of 1020% carbon dioxide, 1020% nitrogen, and the remainder is helium. This laser treatment produces a precise wavelength of light in the infrared spectrum of 10,600 nanometers. The non-ablative mode of fractional CO2 laser beams infiltrates the topmost layers of the skin reaching into the dermis. It produces tiny microscopic thermal abrasions in the tissues that stimulate the regeneration of new collagen while reconstructing damaged skin tissues into new epidermal cells.

Medications For Overactive Bladder

Drugs used to treat OAB block the abnormal contractions of the bladder muscle and can therefore also help ease the symptoms of urge urinary incontinence in both men and women. Youll find a list of OAB medications available in Canada on page 20. Generally speaking, these drugs fall into three categories:

Anticholinergics

Anticholinergic medications block the action of acetylcholine, a chemical messenger that tells the muscles of the bladder wall to contract. Unfortunately, acetylcholine acts in other parts of the body as well, so medications that block it can cause unwanted side effects like dry mouth, blurred vision, impaired cognition and constipation. New extended or prolonged release versions of anticholinergic medications reduce the incidence of side effects and improve compliance because they only need to be taken once a day to produce a steady absorption rate and constant blood levels of the drug.

Examine the efficacy of vaginal Erbium laser treatment for stress urinary incontinence in compare to placebo.

Plenty of women suffer from SUI. It is a condition in which involuntary urine leakage occurs during exercise. This condition affects about 40% of all women and causes huge disturbance to daily life. There are plenty of risk factors, such as age, obesity and parity. We assume that the pathophysiology is related to hypermobility of the urethra due to damage to the surrounding tissue.

The treatment options vary from surgical interventional to conservative, each treatment has its own success rate and risk factors. An attempt to find effective treatment option has been made.

The efficacy and safety of vaginal laser therapy has been proven already, but in the field of SUI the numbers in each study were low and there was no control group.

How Does Laser Stone Surgery Work

The surgery is minimally invasive and requires no incisions on your body. Once you are asleep, a small lighted instrument is placed into your urethra and bladder to access your ureter and kidney.

Once the stone is seen, a laser fiber is used to transmit Holmium energy to break up your kidney stones. The surgeon removes some pieces through the urethra with a small basket, and smaller pieces can be passed later with urination. The surgeon may also use a high-powered holmium laser with high-frequency emissions that “dust” the stones into a fine powder. You can then pass the fine particles in your urine after surgery. An even more efficient way of transmitting the holmium laser is by modulating the pulse using the Moses effect, which may result in more efficient stone dusting or fragmentation.

The surgeon will likely insert a tube called a stent between the kidney and urethra. This promotes healing and lets small stone fragments pass more easily. Most importantly, this stent allows your kidney to drain during the healing process. The stent is removed about 1 week after surgery. The length of surgery is generally under 1 hour, depending on the size and number of your kidney stones. Use of the dusting technique combined with the Moses effect may decrease the need to remove stones with a basket, thereby minimizing the need to use a stent.

Study Design And Characteristics Of The Study Participants

This was a prospective, open-label cohort study conducted in a secondary care facility. Female patients were referred from primary care physicians, and if they met the inclusion/exclusion criteria, they were recruited consecutively between December 2018 and February 2019. The study was approved by the institutional review board, human ethics committee of the faculty of Health Sciences, the University of Warsaw, Poland . A total of 60 women were enrolled and treated at the Estebelle Clinic, Warsaw. Fifty-nine of them completed three treatment sessions with 46 weeks intervals, and 3-, 6-, and 12-month follow-up visits. Only one patient did not attend the 6-month follow-up, a lower-than-expected dropout rate, strengthening the final results from this study.

All study subjects approved the protocol and guidelines which were available in the local language, by signing a written consent. Age range of the treated patients was 3075 years old and the severity of SUI at the entry screening, which was the predominant symptom in all patients, was defined as mild, moderate, or severe, according to the Sandvik score . Comorbidities included hypertension , diabetes , and 5 patients defined as smokers.
