Role Of Alpha Blockers
Acute urinary retention related to BPH may be consecutive to a sudden stimulation of alpha 1 – adrenergic receptors. The rationale for the use of alpha blockers in BPH is that it acts on the dynamic component of bladder outlet obstruction by relaxing smooth muscle fibers located in the prostate and its capsule, bladder neck and prostatic urethra.
The role of alpha blockers in the primary prevention of AUR in symptomatic BPH patients has been well studied. In a six-month placebo-controlled study involving 518 patients IR-alfuzosin was associated with a significantly lower incidence of AUR than placebo . Among 3228 patients treated with IR-alfuzosin for three years, the incidence of AUR with alfuzosin was only 0.3% vs. 2-3% in the watchful waiting group. Emberton M et al analyzed the incidence of AUR in 5,792 men with BPH receiving alfuzosin at 10 mg once daily for six months. The study population included 3.8% patients with prior history of AUR managed conservatively. The rate of AUR and BPH-related surgery during treatment was 0.5% and 1.1% respectively.
