There Are A Lot Of Variables To Consider When Diagnosing A Uti

Women who have had UTIs before most likely recognize their symptoms. However, something that causes you to urinate more frequently and causes discomfort isnt necessarily a UTI.

My first goal when speaking with a patient is to get as much information as I can, so I can determine the best diagnosis and treatment plan, says Dr. Talbott. Urinary tract infections can be confused with sexually transmitted diseases, or they can be more complicated if you have diabetes or other diseases that contribute to a suppressed immune system. It is also challenging when addressing a pediatric patient, or in the elderly with multiple complex health issues.

If youre experiencing fever, nausea, or pain, you most likely have a kidney infection. However, if you have thick white discharge, you might have a yeast infection. Blood in the urine may also suggest a bladder infection, while a discharge with a fishy odor may indicate bacterial vaginosis.

Giving your doctor as much information as you can helps him or her determine the most likely way to get you feeling better fast, says Dr. Talbott.

What Causes A Urinary Tract Infection

Cystex Urinary Pain Relief Tablets

Urinary tract infections are caused by microorganisms usually bacteria that enter the urethra and bladder, causing inflammation and infection. Though a UTI most commonly happens in the urethra and bladder, bacteria can also travel up the ureters and infect your kidneys.

More than 90% of bladder infection cases are caused by E. coli, a bacterium normally found in the intestines.

The Doctors: Visceral Pain

Dr. Travis explained that Visceral Pain comes from the internal organs, but it is not always clear where it is coming from, because it could manifest somewhere else in the body.

That is because of the nerves that run through your body. Thats why, for example, Heart Attack symptoms can include arm pain.

How do you tell the difference? Dr. Travis used Dr. Jim Sears as a model to explain how he examines a patient. To determine the difference between a Pulled Muscle and Appendicitis, he explained that he would have the patient bend the knees and relax the abdomen.

You Can Get Help Without Going To A Doctors Office

In the past, receiving treatment for a UTI required scheduling an office visit and suffering through your symptoms until you can see a doctor. Now, telemedicine services like MDLIVE allow you can talk to a doctor via phone or videowherever you arein an average of 10 minutes or less. And if you need a prescription, you can have it in your hands that same day.

When it comes to UTIs, there are tests a doctor can run a urinalysis and culture, which can get quite expensive. However, if youre a healthy female between the ages of 18 and 65 with the symptoms of a UTI and without any complications, these tests most likely arent going to change the initial treatment a doctor prescribes, says Dr. Talbott.

If your doctor sees anything complicated in your symptoms, it does make sense to do a urinalysis and possibly other laboratory tests. But in a majority of cases, its reasonable to initiate treatment and forgo the cost and inconvenience of testing, and delay in treatment, because most of the time it wont change the antibiotic thats prescribed.

This makes telemedicine a more convenient and inexpensive way to get the treatment you need to minimize your discomfort and start feeling better, faster.*

How Do I Prevent Utis

I believe patient education is central to prevention. The more a woman understands the factors that can increase her risk for infection, the easier infections are to prevent, says Dr. Katherine Klos, a board-certified urologist practicing in Washington D.C. She adds that hydration can also be a preventive measure. The why behind how hydration decreased UTI rate is attributed to two factors: the dilution of urine, which reduces nutrient concentration needed for bacterial growth, and the flushing of bacteria, Dr. Klos says. With more urine volume, you are forced to empty the bladder at shorter intervals, thus pushing opportunistic bacteria out of the bladder instead of allowing it to sit and multiply within the bladder. Further ways to prevent UTIs include peeing after sex, wiping from front to back, and minimizing irritating feminine products in the genital area. While these can be helpful for some people, its not always enough for everybody.

Is An Otc Uti Test Accurate

An OTC UTI test strip is pretty accurate if it is used for screening. The problem with the results arises when you use an over-the-counter UTI test strip to diagnose a UTI.

If you have UTI-like symptoms and rely on the test, youre asking it to do something it was never meant to do. In fact, screening tests tend to underdiagnose if you have symptoms. Thats because a symptomatic person is at such a high risk for the disease that a screening test can miss them.

On the other hand, if you use an OTC UTI test in a group of people who have no symptoms, it will do much better to identify patients with bacterial overgrowth.

This is why UTI tests sensitivity can change from 56% to 92% depending on who is being tested.

Research shows that the role of these tests should be to screen patients without any symptoms. The marketing ploys for over-the-counter UTI strips generally fail to mention this important fact.

Methenamine Hippurate Is A Drug That Sterilises Urine Stopping The Growth Of Certain Bacteria Previous Studies Have Shown That It Could Be Effective In Preventing Utis But The Evidence Is Inconclusive And Further Randomised Trials Are Needed

The current guidelines as the standard preventive treatment for recurrent urinary tract infection recommend daily low dose antibiotics.

However, such long term use of antibiotics has been linked to antibiotic resistance, so research into non-antibiotic alternatives is urgently needed.

Methenamine hippurate is a drug that sterilises urine, stopping the growth of certain bacteria. Previous studies have shown that it could be effective in preventing UTIs, but the evidence is inconclusive and further randomised trials are needed.

A team of UK researchers, led by clinicians and scientists from Newcastle-upon-Tyne, set out to test if methenamine hippurate is an effective alternative to standard antibiotic treatment for preventing recurrent UTI in women.

The findings of the study were published in the journal The BMJ.

Their findings are based on 240 women with recurrent urinary tract infections requiring prophylactic treatment. On average before trial entry, these women experienced over six UTI episodes per year.

Women were recruited from UK secondary care centres between June 2016 and June 2018 and were randomly assigned to daily antibiotics or daily methenamine hippurate for 12 months, with three monthly assessments up to 18 months.

The non-inferiority margin, defined after a series of patient focus group meetings, was a difference of one UTI episode per year.

Summary Of Study Characteristics

presents a table of the main study findings and authors conclusions.

Clinical Effectiveness of Antiseptic Agents or Procedures

The results of the NMA did not show any statistically significant differences in the incidence of CAUTIs between the different cleaning methods. Although the differences in effectiveness were not statistically significantly different, the authors used probability sequencing to rank the interventions for effectiveness from one to seven. Chlorhexidine ranked first, clean water was second, soap and water third. Iodine, saline, routine meatal care, and antibacterial use ranked from four to seven.

The results of the meta-analysis comparing water and antiseptics showed no statistically significant difference in the rate of CAUTI between groups . Subgroup analyses were conducted comparing water with povidone-iodine and water with chlorhexidine and the lack of statistically significant differences in bacteriuria rates remained. Based on the data provided in the individual studies included in the SR, the authors concluded that either tap water or sterile water could be used as an option for periurethral cleaning prior to urinary catheter insertion.

Evidence-based Guidelines

No relevant evidence-based guidelines regarding the use of antiseptic procedures or agents to reduce infection in patients undergoing urinary catheter insertion were identified therefore, no summary can be provided.

For Standard Uti Testing Always Keep A Urine Sample Container On Hand Why

If you frequently experience UTIs, and plan to collect your urine sample at home, youre more likely to get an accurate result if you collect your urine sample at the height of your symptoms.

Symptoms tend to flare up when the concentration of bacteria in your bladder increases, so this is the perfect time to get a snapshot of what is happening in your urine.

If you suffer from recurrent UTIs, ask your doctor or your chosen lab for a sterile sample container to take home or order a UTI test kit online. Make sure this is provided inside a sterile package, to prevent contamination before you need to use it.

Next time you get acute symptoms, pee in that container before you start guzzling fluids, trying home remedies, or popping pills. Drinking excessive amounts of fluids before collecting your sample will only dilute your urine, decreasing the likelihood of an accurate test.

Otc Uti Treatment: Key Takeaway

Women are prone to contracting a urinary tract infection at least once in their life. Certain UTIs do not need treatment if they are diagnosed on time and if the symptoms are cared for, however, some UTIs require medical intervention in the form of antibiotics.

While antibiotics are the standard treatment for UTIs, researchers are looking for better OTC treatment options for UTI symptoms that might eliminate their need. Several OTC UTI treatment drugs help prevent and manage UTI symptoms but should never be considered a replacement to prescribed antibiotics. The only clinically proven cure for a UTI is a prescribed antibiotic and nothing else as of yet.

What About Antibiotic Resistance

Resistance rates for antibiotics are always variable based on local patterns in the community and specific risk factors for patients, such as recent antibiotic use, hospital stay or travel. If you have taken an antibiotic in the last 3 months or traveled internationally, be sure to tell your doctor.

High rates of antibiotic resistance are being seen with both ampicillin and amoxicillin for cystitis , although amoxicillin/clavulanate may still be an option. Other oral treatments with reported increasing rates of resistance include sulfamethoxazole and trimethoprim and the fluoroquinolones. Resistance rates for the oral cephalosporins and amoxicillin/clavulanate are still usually less than 10 percent.

Always finish taking your entire course of antibiotic unless your doctor tells you to stop. Keep taking your antibiotic even if you feel better and you think you dont need your antibiotic anymore.

If you stop your treatment early, your infection may return quickly and you can develop resistance to the antibiotic you were using previously. Your antibiotic may not work as well the next time you use it.

If your healthcare professional prescribes you antibiotics:

  • Take antibiotics exactly as your healthcare professional tells you.
  • Do not share your antibiotics with others.
  • Do not save antibiotics for later. Talk to your healthcare professional about safely discarding leftover antibiotics.
