All types of UTI are more frequent in patients with type 2 diabetes. Various studies have reported the overall incidence of UTI among these patients. An observational study of all patients with type 2 diabetes in the UK general practice research database found that the incidence rate of UTI was 46.9 per 1,000 person-years among diabetic patients and 29.9 for patients without diabetes.24 Women with previously diagnosed diabetes had a higher risk of UTI than those with recently diagnosed diabetes .24 A cohort study of over 6,000 patients enrolled in ten clinical trials found an incidence rate of 91.5 per 1,000 person-years in women and 28 per 1,000 person-years in men, and a cumulative incidence of 2% during 6 months.25 A recent American study performed on a health service data base with more than 70,000 patients with type 2 diabetes found that 8.2% were diagnosed with UTI during 1 year .22 Another American database study from 2014 found that a UTI diagnosis was more common in men and women with diabetes than in those without diabetes among 89,790 matched pairs of patients with and without type 2 diabetes mellitus.26

In men, risk of acute bacterial prostatitis, prostatic abscess, progression to chronic prostatitis, and infections following prostatic manipulations, such as trans-rectal prostate biopsy, is increased in patients with diabetes mellitus.36,37

For the greatest improvement with the fewest steps, do the following:

  • Stop eating all sweeteners. Sugar impairs the bodys ability to fight infection. The worst sweets are likely to be those with high-fructose content such as soft drinks.

Why Would Sugar Make A Difference With Incontinence

It may sound crazy, but what you eat has a big effect on how your bladder behaves. While its different for everyone, there are many foods that can irritate the bladder. These include things like acidic foods , caffeine, and yes, even sugar. Sugar can stimulate the bladder, causing sudden urges, which may lead to leaks. Even natural sugars, like honey, or artificial sweeteners can cause this irritation.

And, lets be honest. Besides just the effect on your bladder, you probably know that sugar is not great for you anyway. It can indirectly lead to number of other conditions, which consequently, may then lead to incontinence.

Too much sugar may lead to more frequent urinary tract infections, a condition that sometimes causes short-term incontinence. The bacteria that cause UTIs love sugar. Sugar makes the acid level of your urine higher, creating a better environment for this bacteria, and causing the infection to grow more quickly.

Eating too much sugar can also lead to weight gain, which places extra strain on the pelvic floor muscles and can cause incontinence. It may also lead to diabetes, a condition where your body has a hard time processing the glucose in your body, causing potentially high blood sugar levels. These high levels of blood sugar can cause a condition called diabetic neuropathy, specifically, neurogenic bladder, which depending on the nerves affected, may cause a bladder to be under or overactive.

Drink Plenty Of Fluids

Drinking water can encourage urination, which helps flush bacteria out of the urinary tract. Your NYU Langone doctor may recommend drinking more water than you typically do.

Some studies have shown that certain substances in cranberries can prevent bacteria that cause a UTI from adhering to the bladder wall. As a result, your doctor may recommend taking cranberry extract supplements or drinking concentrated cranberry juice regularly.

The risk of consuming too much sugar is greater for certain people. In particular, Sugar can increase the risk of UTIs mostly in patients who and are prone to elevated urine and blood sugar levels, Bellin says.

The reason? When there is a high level of sugar in urine, UTI-causing bacteria can grow more easily, per the National Kidney Foundation. Bacteria need nutrients to survive, and sugar can be used as a nutritional source, Bellin says. In other words, dont give the bacteria something to munch on.

Besides the potential for excess sugar to contribute to UTIs in people with diabetes, no one food or type of food will directly cause a UTI, including dairy products like milk or cheese. However, Bellin notes that people who are lactose intolerant and still consume dairy despite the havoc it wreaks on their digestive system may be slightly more at risk for developing a UTI.

If someone is lactose intolerant, dairy can cause more diarrhea and GI issues, which in turn will increase chances that GI flora can get into the urinary tract and cause an infection, Bellin says.

Other Ways To Prevent Some Utis Coming Back

If you keep getting a bladder infection , there is some evidence it may be helpful to take:

  • D-mannose a sugar you can buy as a powder or tablets to take every day
  • cranberry products available as juice, tablets or capsules to take every day

Speak to your doctor before taking any of these during pregnancy.

Be aware that D-mannose and cranberry products can contain a lot of sugar.

If youre taking warfarin, you should avoid cranberry products.

    Cats with diabetes are prone to urinary tract infections , but non-diabetic cats are also at risk. Symptoms of a UTI include the cat urinating in the house , straining to urinate, frequent urination, licking the genital area, and blood in the urine. However, some cat urinary infections will only cause vague symptoms such as decreased activity level, loss of appetite, or vomiting.

    Because diabetes suppresses the immune system, diabetic cats are more susceptible to various infections. Bladder infections are common, because the body dumps excess glucose into the urine, where it provides an energy source for any bacteria present.

    Its important to promptly diagnose and treat feline UTIs, not only for the cats comfort but also because the inflammatory response interferes with the bodys use of insulin. An untreated urinary infection in a diabetic cat can lead to deregulation of the diabetes, diabetic ketoacidosis , pyelonephritis and serious illness.

    If a bladder or kidney infection is confirmed, your veterinarian will prescribe antibiotics. Usually antibiotics are continued for six to eight weeks . The ideal antibiotic will depend on the sensitivity results, but commonly used are amoxicillin/ clavulanate , orbifloxacin and marbofloxacin . The urine culture will be rechecked following completion of antibiotics.

    Are There Other Conditions That May Seem Like A Uti

    We encourage women with urinary symptoms to be seen in the office to confirm the diagnosis. Urinary infections are sometimes mistaken for other conditions and vice-versa. Conditions that cause similar conditions include:

    • Vaginal infections are usually characterized by discharge, irritation, itching, and/or odor, but burning on urination is occasionally the only symptom.
    • Genital herpes can cause burning with urination.
    • Ovarian cysts can cause lower abdominal pain.
    • Bladder stones can sometimes cause blood in the urine.

    With the exception of women who have already had numerous culture-proven UTIs, women who have symptoms suggesting a UTI should be seen in the office to rule out other causes for their symptoms.

    Be Careful About Hidden Sugars

    When I talk about sugar, it’s not just the sugar in your tea or on your cereal, but hidden in the ingredients of things like yoghurt, pasta sauces and baked beans.     10.1.2. Pyelonephritis management in patients with type II diabetes

    Hospitalization should be done for the patients with severe symptoms for initial intravenous antibiotic therapy . Empiric antibiotics treatment: broad-spectrum cephalosporins, aminoglycosides, fluoroquinolones, piperacillin-tazobactam, or carbapenems should be started . Severe sepsis presenting patients or those known to harbor-resistant uropathogens or the patients who have received multiple antibiotic courses should receive broad-spectrum coverage, guided by current urinary culture report. Treatment should be tailored when culture reports are available.
